Job Description
Houston Hospice is seeking an experienced Hospice Clinical Liaison (RN) to serve as a trusted clinical resource to hospitals and healthcare partners. This relationship-driven role focuses on building trust, providing education, and ensuring patients and families access compassionate end-of-life care when they need it most.
What You'll Do
- Develop and maintain strong relationships with hospitals, physicians, case managers, and healthcare partners
- Serve as clinical expert on hospice philosophy, services, eligibility criteria, and referral processes
- Identify patients who may benefit from hospice and facilitate timely evaluation and referral
- Educate patients, families, caregivers, and healthcare professionals about hospice services and admission processes
- Conduct educational presentations and in-services for healthcare professionals and community partners
Requirements
- Graduate of accredited nursing school with current Texas RN license
- Current Basic Life Support (BLS) certification
- Excellent organizational, interpersonal, and written communication skills
- Reliable, insured vehicle and valid Texas Driver's license; ability to travel to multiple work locations
- Ability to function as part of interdisciplinary team and accommodate multi-faith backgrounds

Mid-shift usually involves documentation, family communication, and working with physicians on care plan updates โ often by phone or telehealth rather than in-person rounds.
Depending on the facility, you may also handle admissions paperwork, manage wound care, supervise medication techs, and respond to emergencies. The documentation load in senior living is significant โ state surveys and compliance requirements mean thorough charting is non-negotiable. Many nurses in this setting say the most rewarding part is knowing their residents personally and being able to notice subtle changes that a rotating hospital nurse might miss.
